The Rural Health Clinic (RHC) is located on the lobby level of Salem Township Hospital, 1201 Ricker Dr., Salem, IL. Enter through the new RHC entrance in the front parking lot.
To complete school and sports physicals, parents must be present to provide immunization records and sign physical forms.
