The Rural Health Primary Clinic is located on the lobby level of Salem Township Hospital, 1201 Ricker Dr., Salem, IL. Enter through the Rural Health Clinic entrance in the front parking lot of the hospital.

**To complete school and sports physicals, parents must be present to provide immunization records and sign physical forms. Vaccinations are not included in the physical.
