All bottles of medications you take, including over-the-counter medication, vitamins, and supplements
Please list reasons for taking, dosage, and how/when they're taken.

Choosing a trusted family member or friend as your care partner is crucial. Make sure to have their current contact information available.
A care partner is someone who:
Is an adult (18 years or older) and can stay with you for 24 hours post-op if required.
Will assist you with your care and make decisions about it.
Will be responsible for updating other family members.
Will support your recovery both in the hospital and at home.
Will ensure that your preferences, values, and goals are considered while planning your care.
